Before you book
Submit your Medicaid enrollment application first and get your Application Tracking Number (ATN). After 24 to 72 hours, register in the Clearinghouse portal under Florida Medicaid and sign the user registration agreement.
Once you’re approved, start the screening in the Clearinghouse. It assigns the ORI and produces the Livescan Request Form you bring to your appointment.
At your appointment
Bring a valid photo ID and your Livescan Request Form. We capture your fingerprints and the photo the Clearinghouse requires.
After your appointment
Results go only to the agency you logged in under, so make sure that was Florida Medicaid. When your status shows Eligible for Medicaid Provider Enrollment, print the result and upload it to your application.
Rescreening is required every five years. If you already have an eligible screening with a photo from another agency, Medicaid may be able to review it instead.
